Advance Care Planning Appointments
HCCA hosts appointments with Canberra Health Services where you can complete your Advance Care Plan at the Chifley Health and Wellbeing Hub. Bookings are available on the 3rd Tuesday of each month from 12:30-6pm for 1 hour per person. Call HCCA on 02 6230 7800 or email [email protected] to book.
